Poly-L-lactic acid arrives as a freeze-dried powder that must be reconstituted and allowed to hydrate before injection. Dilution volume, hydration time and placement plane are three of the largest variables separating smooth collagen from a palpable nodule.
A powder that becomes your own collagen
Sculptra is poly-L-lactic acid, supplied as a lyophilized (freeze-dried) powder. It does not add volume on the day of treatment. It is suspended in sterile water, injected into the deep dermal or subcutaneous plane, and provokes a controlled foreign body response that deposits type I collagen over the following months. The result visible at three to six months is your own tissue.
Dilution, hydration time, and placement plane
Preparation matters more than most patients realize. Higher dilution volumes disperse the particles more evenly and are associated with lower rates of papules and nodules than the sparse original protocol. Adequate hydration time before injection allows full suspension, which matters for the same reason. Placement plane is the third variable. Superficial placement, or placement in highly mobile areas such as the lips and immediate periorbital region, concentrates particles where they can be felt and seen.
Technique carries the rest. Product is laid down in retrograde threads or a fanning pattern rather than deposited as a bolus, because a bolus is a concentration by definition. Even distribution across the treatment area is the goal, not filling a specific hollow the way hyaluronic acid would.
