What do density and depth actually mean on a CO₂ laser?

Fractional CO₂ is not one treatment. Density controls what percentage of the skin surface is ablated. Depth controls how far into the dermis the energy travels. Two providers can both call it full-face CO₂ and deliver entirely different procedures, recovery times and results.
How a fractional CO₂ laser works — and what the two settings control
A fractional CO₂ laser emits at 10,600 nanometers, a wavelength absorbed almost entirely by water. It fires thousands of microscopic columns of energy into the skin, each vaporizing a channel of tissue surrounded by a zone of controlled thermal coagulation. The untreated skin between those columns is what makes recovery survivable and drives re-epithelialization from the edges inward.
Two settings define the treatment. Density is the percentage of surface area treated per pass. Depth is a function of energy per microbeam, measured in millijoules, and determines how far into the dermis each column reaches. Higher density means more downtime, more erythema (persistent redness), and materially higher post-inflammatory hyperpigmentation risk in Fitzpatrick IV through VI. Greater depth is what addresses dermal problems: atrophic scarring, deep rhytids, structural laxity.
One aggressive session vs. several moderate ones
The two are adjusted independently and traded against each other, which creates a real strategic choice. One aggressive session delivers more change but demands a week of genuine downtime and carries more pigment risk. Three moderate sessions spaced six to eight weeks apart reach a comparable endpoint with shorter recoveries and a wider safety margin on deeper skin tones. Neither is universally correct, and a provider who only ever offers one has made the decision for you.
A common technical error is stacking multiple high-density passes over the same zone. That converts fractional treatment into bulk heating, removes the untreated reservoir the skin heals from, and raises the risk of prolonged erythema and scarring.
What separates a considered plan from a default one
Two other things separate a considered plan from a default one. Pigment suppression started before treatment in patients at risk, and a test spot in an inconspicuous area when skin type or history is uncertain. Neither takes long, and both change outcomes.
This is why what settings and how many sessions are far more informative questions than what device. Parameters should be set per patient after a skin type assessment, not run at a standing default.
